For the previous two days, a client has had cramping in her stomach, diarrhea, and concentrated pee. A focused assessment would take dehydration indications into account. Weight loss, a reduction in tissue turgor, and dry, cracked mucous membranes are all signs of dehydration.

When a customer has pains in their stomach and diarrhea, their body loses additional fluids. In order to look for a fluid volume deficit, the nurse should do a focused assessment. Dehydration symptoms and weight loss would be indicative of this. An attentive check typically reveals increased bowel noises associated with the cramping. Diarrhea for two days would not suggest renal suppression, but it could cause hypovolemic shock and severe dehydration. Bicarbonate is lost during diarrhea, which could lead to metabolic acidosis instead of alkalosis.
