For someone learning English, why might English idiomatic expressions be difficult to understand?

Idiomatic expressions have a different meaning of the words in the expression. Hold your tongue, hold your horse, etc. This could be confusing because someone would have to learn these expressions to mean something different. But at other times, it can be literal.
