Mind reading, filtering, derailing, judging, and placating are five factors that can hamper and effective communication. Mind reading is when a person assumes to know what the other is thinking about without even bothering to ask the person; filtering is when a person seems to listen but is clearly ignoring some points of the discussion; derailing is when a person suddenly changes the topic of discussion; judging is when a person is too quick to evaluate the other person without even trying to understand how the person feels; and placating is when a person agrees to quickly without even thinking about his decisions. 

These are all blockages to an effective communication which can lead to conflicts and problems to be left unsolved.
