Explanation:

The Imperfect Tense (Preterite) is one of the two forms of simple past in Spanish, and it is used to:

- talk about past habitual actions (adding <u>used to</u> in its different forms)

- describe what someone was doing when it was interrupted by someone (or something) else. In fact, is more a combination of the past tense and a continuous or repeating aspect. Frequently, this includes a sense of incompletion in the verb, but not always. (Is the equivalent to the Past Progresive Tense in english)

In other words:

It is specially used to describe what used to happen or what was happening.

In this sentence we are talking about two simultaneous facts: (1) the arrival of the ambulance and (2) the dizziness of the husband.

Cuando llegó la ambulancia, el esposo <u>estaba </u>mareado

When the ambulance arrived, the husband was dizzy

Note the husband was dizzy is an action that was taking place at that time in the past when the ambulance arrived.

That is why the verb estar (to be) is written in  imperfect tense as estaba (was).
