I would say a), c) and d) all represent potential ethical issues and becoming aware of the mistake and misquote means that they must be reported immediately ie use full disclosure  and for referring a friend for a job interview could be considered conflict of interest.

<span>It could be said that options A  and C  might represent an ethical issue. In the case of A because misquoting might lead to someone be offended by it and you might need to check the document again in order to modify it.As regards C, if somebody detects a flaw should be able to report it to some authorities but sometimes this does not happen and many serious consequences can occur. <span>
