Well, if this is from the giver, the boy's father injects the smaller twin. That is why when the giver actually shows the boy what happened during the process, the boy is so distraught. After Noah translates for the guy a bit because the cop does not speak the same language as him, he learns that the guy is actually pretty nice and was put in jail for shoplifting video games. The deception contributes to the story because it sorta uses the metaphorical phrase don't judge a book by its cover as a theme for this chapter.
