Sorry this is so late:

I'd say that 'I would like ice cream with chocolate and caramel sauce.' (A) is the correct option for this one; since 'chocolate with caramel sauce' is adding more detail on how the narrator would like their ice cream done.

(Sorry if this is worded strangely, I couldn't think of a better way to word it...)
