This one is a hard one and also depends on what your school, teacher, or lesson teaches you how to make a fax cover sheet. I would say the *best answer* is either A or B, even though I am still not completely sure which one, out of the four, it is

The problem with answer A is that certain information is required. Yeah, you have the person you are sending it to and the person knows you, but maybe they still need some sort of reminder of what your case is.

B can be correct, but there might be other information needed other than the simple information requested. It depends if you make the fax cover sheet or the person you're sending it to made it. If they did, then it makes sense.

C I don't believe is correct. I have never heard that.

And although D can be correct, in the real world, it's sometimes hard for a copier to read pencil, and most cover sheets are typed out.

Now that I have explained each answer choice, I am leaning towards B, but I am still unsure. And like I said before, it really depends on what you were taught, because I might have been taught that the answer is B, but you might have been taught that the answer is A. So it really depends. Try to look back at what you learned.
