well you need to fill out an application that has the title, completion year, first publication, etc, then you have to pay the fee and finally upload or mail in a copy of your work. this is the source i used, hope this helps!

Well, I suppose it depends on the Football Clubs Management and their general agreement with the FAs and of course the contract entered with the players themselves. Basically though as far as I know, players receive payment monthly but there are other incentives that come with number of appearances, goals scored, tournaments won etc which is basically an internal arrangements between clubs and players
