The correct one would be " me llamo Isabel" You would have to conjugate the verb llamar. (Which is 'to call') Llamas is in the tú form (the 'you' form).

So the other the first one is saying

My your name is Isabel.

The second says 
Your name is Isabel

The third says
Your my name is Isabel

And the fourth says 
My name is Isabel.
