Can some one please help me ive posted the question several times and no one has helped me identify the italicized phrase as a participial, gerund, infinitive, or appositive phrase. the children, fascinated by the clown, did not want to leave the circus tent.

That's because you did not italicize. However I recently had this question in my English. If the italicized phrase on your question was "/to leave the circus tent/" as mine was the correct answer is Infinitive.
