In the question A, you don't need to put a negative form as in English.

"J'ai mal dormi" is the opposite of "J'ai bien dormi". In English, you say "I didn't sleep well", we say "I slept bad"....

Don't forget that the negative "ne ... pas" or "ne...plus", or "ne ... jamais" stand just before and after the verb. => "Je n'ai pas assez travaillé" Here the ne .. pas" becomes "n'.. pas" because there's an elision.

You can't say "je ne ai pas". because two vowels are following.

In the passé composé tense, the "ne pas" is just before and after the auxiliary. "Je ne travaille pas" --> "Je n'ai pas travaillé" .
