How to say "what is your name ?" and "my name is.." in French ?

If you're talking to someone you don't know, you'll use the "vous de
<span>Politesse" (2nd p. plural) or s.o. older than you. More formal than 'TU'</span>

<u>Comment vous-appelez vous ?</u>

<span>If you're talking to so the same age as you = tu 2nd p. sing.
</span><u>Comment t'appelles-tu ?</u> (not formal) 
<span>
<u /></span><u>s'appeler</u> <em>(present tense)</em>
je m'appelle
tu t'appelles
il/elle s'appelle
nous nous appelons
vous vous appelez
ils/elles s'appellent

note that there is a double  'l" before a mute 'e'

This is the answer to your question:

To ask someone their name, a stranger or someone older than you, ask, "Comment vous appelez-vous?". When asking someone your own age, it's "Comment tu t'appelles?
