When the author comes up with these kind of comments we have to say that is a <span>parenthetical in-text citation, which supports some ideas. There is no way to think it is the source in itself. So the second option is the correct one</span>

This pain is revealed in the lines <em>"His neck ached horribly; his brain was on fire, his heart, which had been fluttering faintly, gave a great leap, trying to force itself out at his mouth. His whole body was racked and wrenched with an insupportable anguish!"</em> <u>This is actually the pain that follows the hanging and not the pain of trying to escape the water</u> (as thought by him).
