therefore

h =  - 2  \:  \: and \:  \: k = 4

y = a {(x  + 2)}^{2}  + 4

when x= 0, y = 3

3 = a {(2)}^{2}  + 4

3 = 4a + 4

a =  \frac{ - 1}{4}

therefore equation of the function

y =  -  \frac{1}{4}  {(x + 2)}^{2}  + 4
