Explanation:

Imo The effect would be of fear but some type of tension, power and demand. Because Holmes has Dr Grimesby Roylott calling out who he is and how he is. Therefore by Dr Grimesby screaming, you can tell the effect he had from Holmes by just the things Dr Grimesby was responding back to Holmes with, how frightened, Threaten, and shock, Dr Grimesby was.

I dont know what to put down for thoughts, as in my own thought or the thought of the character, going in the characters position. I also do not know what type of speech you mean, of it being part of speech or speech as in 1pov or 3pov.
