Apply a water-soluble lubricant to the nares. 
 Nares is simply the medical term for nostrils and the irritation is likely due to the NG tube rubbing the interior of the nares. Applying a lubricant will reduce the irritating friction. re-positioning either the NG tube or the client might cause temporary relief, but would most likely cause another part of the nares to become irritated, compounding the problem. The lubrication deals with the root cause of the irritation.
