Once when Ivan Ilyich was sending him away he even said straight out: "We shall all of us die, so why should I grudge a little trouble?"—expressing the fact that he did not think his work burdensome, because he was doing it for a dying man and hoped someone would do the same for him when his time came. For such people around Ivan, his illness and impending death has become bothersome, and he realized the superficiality of that society.

The first word in quotations does NOT always need to be capitalized. If you are quoting what someone said, it should be capitalized, and so should the title of a book, movie, song, etc., and if the first word in quotations is the first word of a sentence. If you are just quoting a small part of a phrase/song/something that has already been written by someone else, it does not have to be capitalized.
