If there is no time specification, then it means that at all (in life / so far), e.g.

I have been to Spain - I've been to Spain (at all)

I haven't been to Canada - I haven't been to Canada (but I'm still alive and can still go there)

vs.

I didn't go to Canada last year - I didn't go to Canada last year (year has ended) - sentence in Past Simple
