First of all, I think it would do justice to me giving a very short explanation on what a Metaphor is.
A metaphor is a figure of speech that uses a word, often times a phrase, to refer or talk about a word. It states a direct similarity between a word or phrase being used, and the word or phrase being described without using the words "as" or "like."
That being said, I will pick out two of the metaphors in the poem.
1) "my stomach was a melon split wide inside my skin."
2) "I felt the life sliding out of me."
Pneumonia - respiratory system (affects the lungs)
Endometriosis - reproductive system (affects the uterine lining)
Fibromyalgia - muscular system (affects muscles, tendons and ligaments)
Dermatitis - integumentary system (affects the skin)

One of the most important themes of Civil Disobedience is the idea that we have the responsibility as citizens to point out the immoral aspects of our society, even if they are enshrined in the political order and culture of our society. Thoreau feels that society as a whole not only can wrong on moral issues but quite often is. It is the responsibility of each citizen to go against the grain when an injustice is discovered, and take the courageous step to hold a mirror to society to open the people's eyes to this injustice.

I would say that all scenarios that are not too complex could be a basis for a short story, but also in principle, you could shorten and simplify any story to make a short story out of it!
So there is no clear distinction.
But among the options, the better candidates are:

the mysterious disappearance of a cake from the pantry
