This letter is to address an issue I experienced while being serviced by one of your staff at (random grocery store), where I purchased a large amount of goods from (store name) on the (date before the letter was sent).

I am severely dissatisfied with your customer service, for a number of reasons. Firstly, the cashier was extremely rude and impolite, and when I reminded (him/her/other) that I was paying for these goods and service, (he/she/other) began a heated argument. The situation became even more discourteous as my children joined me at the cashier's station.  The cashier, who was identified as (name), then went on to charge me more than I owed and threatened to call the police if I did not vacate the premises. I enclosed a copy of the receipt I was given with this letter.

Unfortunately, the problem remains unresolved. To resolve this issue, I am asking that you (A) provide a complete refund of my purchase, and (B) terminate the employ (name). If not, I am threating legal action.

Please contact me within 7 days to confirm that you will concede with my request.   I have prepared a complaint to be submitted to my legal agencies for investigation.  I will not file the complaint if you resolve the problem within this time period I have indicated.
