there are 2 main forms :

the general slope intercept form is

y = ax + b

so, the equation is

y = -4x + 28

the general slope point form is

y - y1 = a(x - x1)

where a (often called m) is again the slope, and x1, y1 are the coordinates of a given point.

so, we have

y - 4 = -4(x - 6)
