Rising action and exposition since Macbeth's fate is foreshadowed by the witches and his wife begins to form an exposition/ theory and idea about how to make their predictions come true
then rising action with the first murder
In an essay it should only be one I'm pretty sure, but in a narrative as many as it takes to explain a lesson learned. If you were writing a book your conclusion would be whatever comes to you and whatever you feel the characters would do/say/feel.
The answer is "she" -- which was "heo" in Old English. (He, me, and we have all been relatively unchanged since the time of Old English.)
