The patient represents the United States. The doctor is president Franklin Delano Roosevelt, and the caretaker is congress. The bottles represent all the reforms and regulations he put into place with the New Deal to try and fix all the problems in the US. The cartoon shows the opinion that FDR was doing too much and had too many new laws under the New Deal. You can tell because the patient looks worried, and it looks like FDR is overprescribing the medicines.
