Impaired blood flow in the lower extremities due to vascular alterations such as peripheral arterial disease can impede wound healing and lead to extensive tissue loss and lower extremity amputation.

In patients with defects and damaged or missing recipient vessels, revascularization combined with free flap implantation is an option for limb preservation.

METHODS:

Using the neighbour matching algorithm, the investigator performed a 1:1 propensity score matching of 615 LXTR patients and 615 vascLXTR patients. A bivariate analysis of postoperative surgical and medical complications was performed on the results of the propensity-matched cohort.

RESULTS:

  • 5386 patients underwent microsurgical free flap reconstruction of the lower extremities.
  • A total of 632 patients underwent combined (endovascular) vascular intervention and lower extremity free flap reconstruction.
  • Diabetes and smoking were more common in this group, with 206 patients having diabetes (32.6%) and 311 being smokers (49.2%).
  • More patients returned to the operating room in the cohort receiving concomitant vascular interventions (24.4% vs. 9.9%; p<0.0001).
  • The 30-day mortality in patients with concomitant vascular surgeries was 3.5% for him compared with 1.3% with free tissue transfer only (p<0.0001).
