Dental pain of pulpal origin:

  • The dental pulp is a highly innervated tissue that contains sympathetic efferent fibers and primary afferent fibers that are involved in transmitting pain. It is also densely innervated by A-nerve fibres.
  • By activating the smooth muscles that surround the arterioles and so limiting the flow of blood through the pulp, sympathetic efferent fibers regulate the microcirculation of the pulp.
  • Reversible pulpitis, irreversible pulpitis, dentinal sensitivity, and necrotic pulp are the most frequent causes of pulpal pain.
  • Threshold pain is a sort of pulp pain that doesn't cause a reaction until the threshold level is raised. Pulp stimulation may distinguish between touch and pain sensations, but only pain.
  • Pulpal pain falls under the category of threshold pain, in which there is no response until the threshold level is raised. Pulpal responds to chemical, mechanical, and thermal stimulation but does not respond to masticatory pressures.
  • Since pulpal pain does not persist for extended periods of time, it can be distinguished from periodontal discomfort.
  • When stimulated, it is felt, then it goes away and is felt again.
