When you feel threatened even when there is no evidence to support it, such as when someone is watching you or acting against you, you have paranoia. Many people experience it at some point. Even if you are aware that your worries are unfounded, they can still be upsetting if they occur too frequently.

More extreme paranoia is clinical. It's a rare mental health problem where you assume the worst about people even when there is no evidence to support it. You don't believe you are being paranoid since you are quite certain it is real. If someone is actually trying to harm you, it isn't paranoia, as the proverb goes.
