In such a case the person who claims, or the claimant should come up with a proper description of the problem, followed by a short information in the body, that also has proper and valid facts presented briefly. Also be prepared to provide a back-up for the claim.

Also you should enclose copies of relevant materials, along with the above-mentioned things and close the message by lucidly stating what exactly have you done. Do not leave the adjustment up to the receiver and do it yourself.
