Substance abuse tend to lead to deterioration in your brain function. This made you extremely inefficient in your job and it could alter your behavior. Companies tend to see this as liability and would fire you if they find out about your usage

A lot of disease are associated with substance abuse. In worst case scenario, it could lead to organs failures that can cause death. Things such as Infection could also occurs from unsafe needles that addicts use to take their drugs.
