"Alienated" comes from the word "alien" which comes from the Latin word for "other". Basically this means that someone is alone - whether physically separated from others, such as in solitary confinement -  or that they don't have a good relationship with others, such as when they can't communicate well and feel alone despite being surrounded by people.
