The only completed subject among these that makes up a fragment is the last example - the loudest musical instruments.
The other examples don't have subjects at all.

 The fragment '<em>the loudest musical instruments</em>' is made up of a complete subject because the words are  an <em>article</em> (the), <em>adjectives</em> (<em>superlative</em>: loudest, and musical) and a <em>noun</em> (instruments). But it doesn't have a verb to complete the phrase, it's just a fragment. The other fragments A, B and C all have verbs forming <em>predicates</em>, indicating actions of some subjects.
