So a fact sheet is most likely something you received in class, usually relating to a topic, in the form of a packet/DBQ (Document Based Question)

If you have questions (Such as that one) you are meant to have read the sheet with all of the information on it (article/letter/story etc) and then to write down 1 thing from the info sheet, to prove that you read it.
