This nurse is technically a witness to a threat now. i dont think she should intervene but she should tell authorities and since shes a nurse she could find therapists(im probably no help)

Hey there,
From what I can understand,
I can tell that the couple is having a dispute among themselves. And most probably the cause is the husband. 
So the women can actually bring her client and seek help from the councilor to tell her problems or go to higher authorities like the police. This might help her to loosen her burden 
This is what I learnt

These proteins are anchored in the lipid bilayers and only non-polar, hydrophobic amino acid residues would be found in the part of the protein, which crosses the membrane. In the interior of the bilayer, these residues would be hidden from the water solvent and associate with the non-polar lipid tails.
