A report by itself is basically <em>an official paper</em>, a document or sometimes legal paper presenting information in an organized format with a particular purpose and aimed to certain public or audience.

Now, a <em>book report</em> isn't necessarily about the writer's personal opinions on the book, like a review is, instead, it's basically <em>a formal essay</em> that talks about and summarizes the contents of the book, including the book's tittle, author, the plot and characters, and usually is part of school's assignment to demonstrate the understanding of it by the student.

Thereby the most appropriate usage on a book report for an English teacher is: <em>"formal"</em>.

Formal, because you would have to cite your sources correctly, use standard punctuation, use standard spelling, avoid contractions, have a thesis statement, and (usually)write in the third person.
