The slope-intercept form of a line is y = mx + b, where m is the slope and b is the y-intercept, x and y are coordinates of a point.  You have the slope in both and a point, so it's your job to fill those into the line equation and solve them for b, like this:
y = mx + b for the first point: -1 = 2(2) + b and - 1 = 4 + b and 5 = b.  Now fill that b value back in with the slope to get that the line is y = 2x + 5.
Now for the second point:
-3=- \frac{1}{2}(-2)+b and
-3= 1 + b and b = -4. Now fill that b value back in with the slope to get the line
y=- \frac{1}{2} x-4
