The nurse must report the sound with all its features explicitly.
She should indicate in which part is heard it, in this case "apparently in the right middle lobe of the client", also should report what time of bheath is heard it, in addition to comparing the sound using the stethoscope and without.
Once written in the report, she should inform the doctor about the case to discuss the following procedure or treatments.

The nurse should document the finding as coarse crackles.  

Coarse crackles refer to low-pitched bubbling moist sounds, which are illustrated as differentiating Velcro. The fine crackles are short and high-pitched popping sounds heard at the time of inspiration and not cleared with coughing. Sibilant wheezes refer to high-pitched musical sounds. The sonorous wheezes refer to low-pitched moaning or snoring sounds.
