<span>First the nurse would describe the "S" or situation, or who she is and patient/situation information. She would then give "B" or background on the patient including related medical history. She would then give "A" or assessment of she/he the nurse has assessed in regard to the problem. She would then give "R" or her recommendation as to what she believes the doctor will want to order to fix the problem.</span>
