"it would be a good idea if you phone the secretary and ask her as semi formal."

MISTAKE:- There you used asked but there will be ASK as we know that what form of verb we use before AND the same form we use after And. Here you used 1st form before And so after And we will use the first form of verb which is ASK .

the only non-formal part is using phone as verb. it is okay but not very formal.

as semi formal, i would say "it would be a good idea if you can call the secretary on phone and ask her."
