If not an accident, say that there was some water leaking from a pipe that went on the stairs or someone was cleaning them and left some hazardous slippery chemicals- that would be a violation of basic workplace standards.
However, our real problem here is Ron. He picks up Ana, and instead of calling someone who could help or seeing if someone else had a first aid kit, he uses a paper towel. The problem with this is that the cut could get infected because the paper towel may have bacteria on it. This is the main violation.

We see Dr. Jekyll as a highly ranked man according to the society's values and beliefs, being very educated and wealthy. He, like many others is not actually content with his position in the society because he has to hold back his urges. Showing individualism and emotions was forbidden in the Victorian society, which is something he craved, yet wasn't allowed because it would ruin his position. This is the main reason why he started turning into Mr. Hyde. It was like a valve that he could use to blow of some steam without ruining his reputation.
