Whenever there is a blow to the head which results in the jarring of the brain this is called a concussion.
A concussion can result from either falling, striking the head against a hard surface, being hit on the head due to collision or a punching, etc.
In the given situation the client is feeling disoriented and restless after suffering from concussion and hence, the highest priority in the nursing diagnosis should be given to assess the risk for any injury that might have occurred following the concussion.
This is because following a concussion that has led to the trauma to the brain there is a high probability that the brain will have an injury either while recovering or at the moment and hence, the symptoms such as restlessness and disorientation should not be ignored.
