According to the Institute for Healthcare Improvement (IHI), the National Patient Safety Foundation (NPSF) Lucian Leape Institute and NORC at the University of Chicago 41 percent of adults in the United States have experienced a medical error in their own care or in the care of someone close to them. A wrong diagnosis is the most commonly type of error reported.
