Cathy is addicted to cigarettes which already puts her at high risk of developing health problems. All her friends smoke which adds on additional peer pressure and negative environment making it difficult for her to quit. Smoking is also an expensive habit.

Meryl has diabetes which she seems to have inherited. She is at a high risk of developing complications resulting from her diabetes and medical treatment for her diabetes alone is expensive.

Paul is being exposed on a daily basis to polluted air. The only thing he could do about that would be to move but that is expensive. He can't do anything about the coal plant or the pollution and he is at high risk of developing respiratory problems.
