- any allergies

- any problems with medication in the past

- any other perscriptions

Explanation:

you should state these things so your doctor does not give you a medication that could harm you, and potentailly die. You should also state if any meditcation like (exapmles) Trazodone, Flouxentine, etc. if you had problems with them so they can assure you won't have anything simialr in your medication like the ones stated.

1. I have visited my provider a month ago and I was treated with the therapy of Pepto-Bismol and since I have sometimes stomach pains he said that I should also take sometimes Panadol and said that I should drink Chamomile tea.

2. I like the best that he was trying to help with other natural things, not only medicine.

3. I didn't like that I didn't have enough information about given medicine.

4. I think that he is good at his job and he is familiar with my condition for a long time.
