Most poems include the title in the body of the poems. It may be in the first stanza or repeated all throughout the poem.

In this case, "To the doctor who treated the raped baby and who felt such despair", the title is not included in the poem. The readers only knew that the poem is for the doctor because of the title, had the title been not worded as such, readers would only infer that the poem is about doctors in general.
