The topic of federalism is an important component of the Health policy debates. People who support the reform want it to be the states’ responsibility. But the unresolved topic of the debate still is the criteria for allocating health policy authority in states or federal government.  

Health care coverage on states hands often times can’t provide it or sustain it, and the federal government intervenes when it is needed.

However, with 60% of the region's population now living in cities, the Middle East is moving toward a more urban lifestyle. People who live in rural areas make a living by working in agriculture as farmers. City residents are focused on the manufacturing and in the service industries. In rural areas, many people live with their extended families as more people move to the large cities, however, these extended families are beginning to break apart. smaller families are more common in the cities.
