An example is:

"Discovered the cure for the disease."

The text alone makes no sense because it leaves the readers with thought of "who discovered the cure for the disease?"

When writing formally, you should avoid sentence fragments completely. There's no penalty for using them but in actual sense, they lack the clarity of complete sentences, and they sound less professional.

A better way to present the example above is

"The scientists discovered the cure for the disease."
