Answer: The counselor should refer the client to a more competent counselor to provide treatment to the client.

Since the counselor have not seen a client harm his or herself before and doesn't have the training what's so ever to handle clients that exhibit that kind of serious behavior. He shouldn't hesitate to refer the client to a more competent counselor that has handled such cases and that was trained to handle clients that harm themselves and behave in such serious Manner.
