A metaphor is used to compare two unlikely things without using the words, like or as. Here, the woman's bright smile is being compared to a light bulb!
That the person is nervous, restless and is probably uncomfortable. I think the tone would be tense and maybe a little agitated? the person is probably worried about something. sorry if this isn't what you needed but I hope it helps :)
