Well this should be a self explanatory answer> its very important to have well written paper work due to the fact that you can be sued. You are clearly using pts identity as well as personal info in these papers. All medical paper work should be well written even if its not payment specific. However for payment reasons it needs to be addressed what was used so the pt can be properly billed.
