Or if answer number 4. was 'Spousal abuse' 
Then it could be known as 'Spousal abuse'
It would not be neglect since he/she is not being denied anything nor being rejected. 
Nor will it be known as insult since he/she is receiving physical abuse, since when you are insulted you are being verbally abused.
